Harmony is closing

We would like to thank all of our patients for being a part of Harmony Health Medical over the past five years. It has been our great privilege to care for you and your family. 

Below, you will find information that will assist you in transitioning to a new health care provider.

What is happening?

After very careful consideration, we have decided to close Harmony Health Medical permanently. This was a very difficult decision but one that was necessary for our leadership team. 

The closure of Harmony Health Medical will happen in three phases. First, we will continue to operate clinically until May 29, 2025. After this date, no further appointments will be scheduled and all appointments currently scheduled for after May 29 will automatically be cancelled. Second, all patients will have 90 days to find a new provider. During this phase from June 1 until August 31, we will continue to take and fill prescription refill requests. Lastly, the administration will remain until the end of the year to process all open insurance claims, to send statements, and collect on all outstanding balances.

We strongly encourage you to begin looking for and secure a new provider by August 31, 2025.

What do I do now?

We understand that this may be a difficult transition. For this reason, we will provide you with on going support until August 31, 2025.

You may not be able to get an appointment for as much as 6 to 8 weeks, so it is very important that you begin looking for a new provider immediately. We recommend that you complete the following steps before the end of May:

  1. Contact your insurance carrier to get a list of IN NETWORK providers in your area.
  2. Research the list of providers to find one that meets your needs.
  3. Schedule an appointment with your chosen provider.
  4. Fill out the release of records request form and send it to us. We will send a copy of your records to your new provider.

Is there a list of local providers?

We do not have a comprehensive list of providers for each area of central Tennesse. The list would be very long. We suggest starting with a simple Google search for “psychiatry near me” to get a list of providers in your area.

The following list is a handful of the practices that are physically close to our office on Dow St in Murfreesboro.

What happens until I can find a new provider?

May 29, 2025, will be the last full clinical day at Harmony Health Medical. Until then, we will continue to operate as we have for the past five years. 

If you have a non-controlled substance, you will be given a 90-day refill of your prescription to ensure that you have enough medications until you have a new provider. If you have controlled substances, you will be given a 30-day refill and you may call our prescription line to get monthly refills until August 31, 2025.

Where is my provider going?

The Harmony Health Medical team will be transitioning to new roles over the next 90 days as well. 

You have the option to continue to see our providers who will be transitioning to new outpatient practices. Below is the information to contact these practices to establish care with them.

Harmony Health Medical is Cynthia Walker’s private practice. She will be transitioning to working at facilities and will no longer practice in an outpatient setting.

Kandra will be joining Personal Growth Counseling, a practice that is located in Cookeville. Although Kandra is a therapist, Personal Growth Counseling also provides Psychiatry services. You may schedule with Kandra or their providers at:

How do I request records for my new provider?

Authorization for Release of Information

Download the Authorization for Release of Information form. When you fill out the form, please include all of your information on the first page and select what portions of your record to send to the new provider. On the second page, fill out ALL of the new provider information. An incomplete form will create delays in sending your records to the new provider.

After you have completed the form please send it to us. You may fax it to 615-896-3187, or send it via postal mail to 1120 Dow St, Murfreesboro TN 37130.

All record requests may take up to 5 business days to complete. We thank you for your patience.
